Presbyterian Senior Care (HMO) and Presbyterian MediCare PPO | Summer 2021 | Your Story

8 Improving your experience Presbyterian Health Plan (PHP) members are at the center of what we do. Your experience—how you feel— as a valued PHP member is important to us. We conduct surveys to hear what members, just like you, are saying. We value your feedback. When you receive a survey from us, it is really important you participate. We use the feedback to improve things for you. Using some of your survey responses, we hope we can continue to improve your experience with us. Some things we are working on for you are: ● Coordinating how we communicate with you ● Making things more valuable for you ● Improving new member onboarding processes ● Improving your experience with our customer service staff ● Improving our website ● And improved telehealth options Some of these changes you will see over the next year. We hope these changes will help your overall healthcare experience. We appreciate your feedback and look forward to hearing from you. If you would like to share your thoughts, please email feedback@phs.org . We want you to know that we are listening. Your voice, your plan You may wonder how to make sure your family knows your wishes if you are injured or sick and can’t speak for yourself. An advance directive helps to make your wishes clear so that they are followed. An electronic Advance Directive, or e-AD, makes it easy to share your wishes with your loved ones and healthcare providers. Making a plan is important so your loved ones don’t have to make crucial decisions in unexpected or emotional situations. Your e-AD document: ● Goes directly into your electronic medical record in real time ● Is easily shared with family, loved ones, and decision-makers ● Leaves no guessing or tough decisions Why flu shots matter so much: Set your reminder! It’s a tiny needle—with a lot of power. A yearly shot is your best defense against the flu, according to the Centers for Disease Control and Prevention (CDC). Why is it so crucial? The flu can do more than make you feverish, achy, and miserable. It can lead to life- threatening infections like pneumonia. Every year, thousands of people end up in the hospital or die from what started as the flu. Step up and stay safe The CDC recommends a yearly flu shot for everyone six months and older. It’s especially important if you have a chronic health problem—such as asthma, COPD, heart disease or diabetes. The flu can make your condition worse. And you have a higher risk of serious complications. It’s best to get your shot before the end of October when flu outbreaks tend to start. Flu viruses can go around for months—even into the spring—so getting a shot later is still a good idea. If you have questions about your electronic advance directive or want to make an appointment with an advance care planning facilitator, please call us at (505) 559-7226 or toll-free at 1-866-773-7226 . If you already have an advance directive on file, it’s easy to review your wishes at phs.vyncahealth.com or go to your myPRES or MyChart account.
